Respiratory viral infection of the neonatal period is highly contagious. Rapid and accurate diagnosis is important for proper treatment and prevention. However, the existing diagnostic method, respiratory virus cell culture, takes a long time to diagnose. Recent development of rapid diagnostic methods such as multiplex reverse transcriptase polymerase chain reaction (RT-PCR) enable early detection and effective treatment of respiratory viral infections. We compared the efficiency of multiplex RT-PCR and R-mix virus culture for rapid detection of respiratory viruses.
We retrospectively analyzed the clinical features and results of R-mix virus culture and multiplex RT-PCR with nasopharyngeal aspiration specimens in 117 newborns admitted to neonatal intensive care unit suspected of infectious diseases.
R-mix virus culture was positive in 29 cases (24.8%) and RT-PCR in 86 cases (73.5%). R-mix virus culture and multiplex RT-PCR were identical in 54 cases (positive 26 cases, negative 28 cases). Among 75 cases that showed different results, 60 showed negative result in R-mix virus culture and positive result in multiplex RT-PCR, and three showed positive result in R-mix virus culture and negative result in multiplex RT-PCR. Different viruses were detected in the remaining 12 cases by both methods.
Multiplex RT-PCR is faster than R-mix virus culture and has the advantage of identifying new respiratory viruses. On the other hand, Multiplex RT-PCR is more susceptible to false positives and mixed infections than R-mix virus culture, so more attention is required when interpreting test results.
신생아는 다른 연령에 비하여 세균감염에 취약하고 세균과 바이러스성 감염질환을 임상양상만으로 구분하는 것이 어려워 감염질환이 의심되는 경우 입원하여 항생제 치료를 우선 시작하게 된다. 그러나 신생아일지라도 감염질환의 상당 부분이 세균감염이 아닌 바이러스 감염으로 인해 발생하므로 항생제 치료 못지않게 증상에 맞는 보존적 치료와 신속한 진단을 통한 질병 전파의 차단 또한 중요하다. 호흡기 바이러스 감염의 경우 기침, 콧물 등 가벼운 상기도감염의 증상만 보이기도 하고, 경우에 따라 호흡곤란으로 기계호흡을 요하는 심각한 증상을 거쳐 사망에 이르는 중증 호흡기 감염을 일으키는 경우도 있다[
신생아의 호흡기 바이러스 감염은 신속한 진단이 중요하다. 그 이유는 호흡기 바이러스 감염은 전염력이 강해 쉽게 질병이 확산될 수 있으므로 신속한 진단을 통해 적절한 치료를 제공하고 질병 확산을 조기에 예방할 수 있기 때문이다. 이에 저자들은 감염질환이 의심되어 순천향대학교 천안병원 신생아집중치료실에 입원한 신생아의 임상양상을 분석하고, 감염의 원인에 대한 검사로 시행하는 호흡기 바이러스 검사법인 바이러스 배양법(R-mix virus culture)과 다중 역전사효소 연쇄반응법(multiplex RT-PCR)에 의한 호흡기 바이러스 진단법의 결과를 비교해보고자 하였다.
본 연구는 순천향대학교 천안병원 연구윤리심의위원회의 심의를 거쳐 대상자의 의무기록을 후향적으로 조사하였다(IRB approval no., 2018-06-042).
2010년 12월 1일부터 2013년 12월 31일까지 순천향대학교 천안병원 신생아집중치료실에 출생 후 30일 이내에 감염질환이 의심되어 입원한 환자들 중 호흡기 바이러스 검사를 시행하였던 117명의 환자의 의무기록을 후향적으로 분석하였다. 신생아들은 입원 당시 증상과 진찰소견, 흉부 방사선 소견을 근거로 하여 호흡기 감염질환으로 정의하였다. 흉부 방사선검사상 폐 침윤을 보이거나 청진 시 수포음이 들리는 경우는 폐렴으로, 청진 시 호기성 천명이 주로 들리고 흉부 방사선 소견이 정상이거나 과팽창을 보이며 빈호흡이 있는 경우는 세기관지염으로, 청진 및 흉부 방사선검사에서 특이점이 없고 비점막 충혈과 점액성 비루, 수유곤란이 있는 경우는 비염으로 진단하였다[
입원 당일 또는 입원한 다음날 비인두 흡인(nasopharyngeal aspiration)을 통한 검체로 호흡기 바이러스 검사를 시행하였다. 검체의 채취는 무균 카테터를 점액 추출기에 연결시킨 후 환아의 콧구멍을 통해 약 2–3 cm 정도 삽입한 후 음압 60 mm Hg하에 흡입하여 비인두액을 채취하였다. 검체는 4°C에 보관되어 검사실로 수송되었으며 parainfluenza virus, RSV, influenza virus type A, B, rhinovirus, adenovirus에 대해 R-mix virus culture (Diagnostics Hybrids Inc., Athens, OH, USA)를 시행하였고, adenovirus, rhinovirus, parainfluenza virus type 1, 2, 3, influenza virus type A, B, RSV type A, B, human metapneumovirus, human coronavirus 229E, OC43에 대해 multiplex RT-PCR (Seeplex RV detection kit; Seegene, Seoul, Korea)을 시행하였다.
R-mix virus culture와 multiplex RT-PCR에 대한 민감도와 특이도, 양성 및 음성 예측도를 산정하여 95% 신뢰구간(confidence interval)으로 표시하였다. 통계분석은 IBM SPSS ver. 25.0 (IBM Corp., Armonk, NY, USA)을 이용하였고, 두 진단검사 간의 민감도와 특이도 비교는 McNemar 검정법을 사용하였으며 분석결과 유의수준은 P-value <0.05로 하였다.
2010년 12월 1일부터 2013년 12월 31일까지 순천향대학교 천안병원에 신생아집중치료실에 감염질환이 의심되어 입원한 신생아 중 호흡기 감염이 의심되어 호흡기 바이러스 검사를 시행한 환자는 총 117명이었다. 대상 신생아는 남녀 75:42명이었고 출생 후 10일 이내에 증상이 발생하여 입원하였던 신생아는 9명(7.7%)이었고, 11일에서 20일 사이 신생아는 51명(43.6%), 21일에서 30일 사이 신생아는 57명(48.7%)으로 대부분 출생 10일 이후 증상이 발생하여 입원한 신생아였다. 신생아 중 21명(17.9%)은 산후조리원에서 증상이 발생한 경우였다. 주 진단으로는 폐렴이 48명(41.0%), 모세기관지염이 49명(41.9%), 패혈증 16명(13.7%), 뇌수막염 11명(9.4%), 위장관 감염 3명(2.6%), 상기도 감염 3명(2.6%), 기타질환 3명(2.6%)이었다. 증상으로는 발열 41명(35.0%), 기침 92명(78.6%), 천명 24명(20.5%), 빈호흡 53명(45.3%), 콧물 87명(74.4%)으로 대부분 기침과 콧물이 주증상이었다(
R-mix virus culture는 117개의 검체 중에서 29개(24.8%)가 양성이었다. Rhinovirus가 19예(63.3%)로 가장 많았으며 RSV 6예(20.0%), adenovirus 3예(10.0%), influenza type A 1예(33.0%), parainfluenza virus 1예(3.3%)였다. 1개의 검체에서는 adenovirus와 rhinovirus가 동시에 검출되었다. 총 29개의 검체에서 30예의 바이러스가 검출되었다(
Multiplex RT-PCR은 117개의 검체 중에서 86개(73.5%)가 양성이었고, 그중 8개(6.8%)의 검체에서 2가지의 바이러스가 동시에 검출되었다. 검출된 바이러스에서 RSV 73예(77.7%), rhinovirus 12예(12.8%) 순으로 가장 많았으며 이외에 influenza virus type A 2예(2.1%), parainfluenza virus 5예(5.3%), human coronavirus 229E 2 예(2.1%)가 검출되어 86개의 검체에서 94예의 바이러스가 검출되었다(
R-mix virus culture와 multiplex RT-PCR을 비교하면 총 54예(양성 26예, 음성 28예)에서 두 결과가 일치하였다. 두 검사에서 모두 양성을 보인 26예 중 12예에서 서로 다른 종류의 바이러스가 검출되었다. R-mix virus culture에서 음성이었으나 multiplex RT-PCR에서 양성이었던 60예 중 R-mix virus culture에서 검출 가능한 호흡기 바이러스(influenza virus type A와 B, parainfluenza virus type 1, 2, 3, rhinovirus, RSV type A와 B, adenovirus)에 양성을 보인 것은 58예였으며, multiplex RT-PCR에서 추가로 확인할 수 있는 바이러스(human metapneumovirus, human coronavirus 229E와 OC43)가 검출된 예는 2예(human coronavirus 229E)였다(
호흡기 바이러스 질환의 진단을 위한 R-mix virus culture의 민감도와 특이도는 29.2%, 89.3%, 양성예측도와 음성예측도는 89.7%, 31.8%였다. 그리고 multiplex RT-PCR의 민감도와 특이도는 88.4%, 58.1%, 양성예측도와 음성예측도는 85.4%, 64.3%였다. 호흡기 바이러스 감염에 의한 질환에 대해 R-mix virus culture는 특이도가 통계적으로 유의하게 높았고(P=0.016), multiplex RT-PCR은 민감도가 통계적으로 유의하게 높았다(P=0.000) (
본 연구는 순천향대학교 천안병원 신생아집중치료실에 감염질환이 의심되어 입원한 생후 30일 미만의 신생아에게서 호흡기 바이러스 감염을 진단하기 위한 진단법인 R-mix virus culture와 multiplex RT-PCR의 결과를 비교하였다. 소아의 호흡기 바이러스 감염에 대한 연구는 다수 보고되었으나 신생아를 대상으로 한 연구는 그 사례가 적으며 대부분 신생아집중치료실에 입원한 미숙아를 대상으로 한 연구였다[
주요 호흡기 감염 바이러스에는 RSV type A와 B, rhinovirus, influenza virus type A와 B, parainfluenza virus, adenovirus 등이 있으며, 하기도 감염으로 입원한 소아의 40% 이상에서 이들 바이러스가 검출된다. 특히 90일 이하의 어린 영아에게서는 rhinovirus와 RSV의 검출률이 높게 보였다[
본 연구에서 R-mix virus culture에 비하여 multiplex RT-PCR에서 양성률이 높았다. R-mix virus culture에서의 양성률은 24.8%로 이전 연구된 보고와 유사한 양성률을 보였다[
2가지 이상의 바이러스가 검출된 것은 R-mix virus culture에서는 1예(3.4%)에서 RSV type A와 B가 중복감염으로 나타났고, multiplex RT-PCR에서는 8예(9.3%)에서 나타나 multiplex RT-PCR에서 중복감염의 빈도가 높았다. Woo 등[
R-mix virus culture에서는 rhinovirus가 19검체(63.3%)에서 확인되어 가장 우위를 차지하였으며 multiplex RT-PCR에서는 RSV가 73검체(77.7%)에서 확인되어 가장 우위를 차지하였다. R-mix virus culture에서 rhinovirus 양성을 보였던 19검체의 multiplex RT-PCR 결과에서는 6검체에서 RSV가 확인되어 서로 다른 결과를 보여 해석에 어려움이 있었다. 신생아에게서는 rhinovirus와 RSV의 중복감염이 많으나 검체의 채취 및 수송과 취급의 문제로 R-mix virus culture와 multiplex RT-PCR의 결과가 다르게 보였을수도 있을 것으로 여기며 신생아에게서의 rhinovirus와 RSV 감염에 대한 후속연구가 필요할 것으로 생각되었다.
R-mix virus culture의 민감도는 29.2% (24.2%–31.7%), 특이도는 89.3% (73.3%–97.1%)로 다른 연구에서 민감도 95%–100%, 특이도 97%–100%이었던 것에 비해 민감도가 낮았다[
R-mix virus culture의 경우 검사결과를 확인하는 데까지 5–7일의 시간이 소요되었으며, multiplex RT-PCR의 경우 2–3일의 시간이 소요되어 바이러스 감염의 전파에 대한 보다 신속한 대처를 할 수 있었다.
이번 연구의 한계점으로는, 첫째, 입원 환자를 대상으로 했기에 경미한 감염을 가진 환자가 누락되었고, 둘째, 바이러스 감염에 영향을 줄 수 있는 모유수유 여부, 간접흡연, 형제 수 등 환경적 요인과 혈액검사결과, 입원기간, 치료방법 등 다양한 임상양상과 비교하지 못하여 중복감염과 서로 다른 결과를 보인 경우에 대한 해석에 어려움이 있었다.
결론적으로, 신생아 호흡기 바이러스 감염질환의 진단에 있어 민감도가 보다 높으며 보다 신속하게 진단하여 질병의 전파를 막기 위한 방법으로는 multiplex RT-PCR이 유리할 것으로 생각되지만, 위양성률이 R-mix virus culture에 비하여 높아 실제 호흡기 바이러스 감염이 아닌 경우가 있을 수 있으므로 주의를 요하며, 여러 가지 바이러스에 대하여 양성 반응을 보일 경우 중복감염과 위양성을 감별하여 결과 해석에 주의하여야 할 것이다.
