소아 대퇴골 간부골절에 대한 유연성 골수내정을 이용한 내고정술 후 하지부동 및 각형성의 발생

ABSTRACT

Objective

Femoral fracture is one of the most frequent fractures in children and complications occurs such as malunion and leg length discrepancy after internal fixation using flexible intramedullary nail. The author intended to consider operational treatment using flexible intramedullary nailing through a quantitative data including age, pattern and location of fracture for angulation, and leg length discrepancy.

Methods

A retrospective survey was performed by 30 cases of child patients who underwent internal fixation using flexible intramedullary nail for femoral shaft fracture. Using a simple radiography, duration of union and angulation were measured. Leg length discrepancy was evaluated by scanogram. By measuring each 2 times at an interval of 1 week by 2 observers, error among observers and error in observer were excluded. For statistical validation, t-test and lineal regression analysis were used.

Results

Immediate postoperatively, valgus condition of 7 degrees was represented and affected side was represented to show varus of 5.6 degrees on average in 46.6% of child patients. The more age of patient is young, more angulation was represented to be significantly taken place in transverse fracture rather than oblique fracture, but depending on fracture location, significant difference was not observed. Leg length discrepancy showed overgrowth of 6.39 mm on average. And cases of overgrowth over 5 mm being were represented and age of all the child patients was below 9 years old. As a result of statistical analysis, it was observed that the age is younger, leg length discrepancy was prone to be occurred, and its significant relevance with fracture form or fracture location was not represented.

Conclusion

Transverse fracture is represented at the time of performing internal fixation using flexible intramedullary nail. Occurrence rate of angulation and leg length discrepancy was high but follow-up period is required to be extended even after the time when growth is completed on a long-term basis and in case of imperfect correction, additional treatment such as physeal plate stapling is required. Bone shortening and lengthening may be required and it is considered to be a useful method for femoral shaft fracture of children.

서 론

대퇴부 골절은 소아에서 가장 흔한 골절 중 하나이며[1] 문헌에 따라 이견은 있으나 4세에서 12세 사이의 연령에 대하여 유연성 골수내정을 이용한 내고정술은 강직성 골수내정에서 발생할 수 있는 무혈성 괴사 등의 합병증이 없어 소아의 하지 간부골절의 효과적인 치료방법으로 인정되고 있다[2-4]. 그러나 유연성 골수내정을 이용한 내고정술 후에 부정유합, 하지부동 등의 합병증이 발생할 수 있다고 보고하고 있어[5-7] 이에 저자는 수술 후 발생하는 하지부동과 각형성에 대해 나이, 골절의 형태와 위치 등의 정량적인 자료를 통해 유연성 골수내정을 이용한 수술적 치료에 대해 고찰하고자 하였다.

대상 및 방법

1. 연구대상 및 방법

2010년 6월부터 2013년 12월까지 순천향대학교 천안병원에서 대퇴골 간부골절에 대하여 수술적 치료를 시행 받은 환아 30명(남아 18명, 여아 12명)을 대상으로 영상학적 검사결과를 이용해 후향적 조사를 시행하였다. 환아의 평균 연령은 7.3세(범위, 4–12세)였고 횡골절이 14예(46.7%), 사골절이 16예(53.3%)였으며, 근위부 1/3의 골절이 12예(40%), 중간부 1/3의 골절이 14예(46.7%), 원위부 1/3의 골절이 4예(13.3%)였고 모두 비관혈적 정복술 후 유연성 골수내정을 이용한 내고정술을 시행하였다. 횡골절은 골절편 간의 각도가 30도 미만인 경우, 사골절은 골절편 간의 각도가 30도 이상인 경우로 정의하였다[8] (Fig. 1). 수술 후 골유합은 정형외과 전문의가 단순방사선영상(전후면, 측면)에서 4개 피질골 중 3면에서 피질골 사이를 연결하는 가골이 완성되었을 때로 정의하였다[9-12].
정량조사는 의무기록과 주사촬영법을 통한 영상학적 자료를 이용하였으며, 하지부동을 측정함에 있어 6세 미만의 경우는 원격방사선촬영법을, 6세 이상의 경우는 주사촬영법을 이용하는 것이 최적기준으로 되어 있는 점을 참고하여 6세 이전에 수술을 시행한 경우 6세가 지난 이후에 주사촬영을 시행하였고 주사촬영상에서 대퇴골두의 중앙에서 경골 천장부 중앙까지의 거리를 측정하여 평가하였다. 각형성의 측정은 수술 직후와 마지막 추시일의 단순방사선영상(전후면)에서 골절면을 기준으로 근위부 장축과 원위부 장축이 이루는 각도를 측정하였다(Fig. 2). 촬영시기 및 추시기간은 수술 후 평균 20.3개월(범위, 13.1–29.1개월)이었고, 2명의 관찰자가 1주 간격으로 각각 2번씩 측정하여 관찰자 간 오차와 관찰자 내 오차를 배제하였다. 통계적 검정 시에는 T검정과 선형회귀분석을 이용하였고, P<0.05를 유의한 것으로 정하였다. 통계분석은 SPSS ver. 14.0 (SPSS Inc., Chicago, IL, USA)을 이용하였다.

2. 증례

교통사고 이후 발생한 좌 하지 통증 및 부종으로 내원한 7세 남아로 대퇴골 간부골절 진단하에 비관혈적 정복술 및 유연성 골수내정을 이용한 고정술을 시행하였다(Fig. 3).

결 과

총 30예의 환자들 모두 추시기간 중 영상학적 골유합 소견을 보였으며, 골유합 시기는 평균 4.9개월(범위, 2.8–6.3개월)이 소요되었고 수술 후 평균 6.8개월(범위, 4.7–8.7개월)에 내고정물을 제거하였다.
각형성은 수술 후 평균 110.6도(범위, 93.5–127.8도)로 건측(평균 103.6도)에 비해 평균 7도 가량 외반된 상태로 나타났으며 마지막 추시상 99.8도(범위, 90.7–113.1도)로 관찰되었고 최종 추시상 환측과 건측의 차이는 환측이 평균 5.6도 내반(범위, 7.3도 외반–16.8도 내반)된 것으로 나타났다. 환아의 46.6%에서는 5도 이상의 각형성을 보였으며 연령이 어린 환자일수록(t=-3.054, P= 0.005), 횡골절에서 사골절보다(t=-4.266, P= 0.001) 유의하게 최종 추시상 각형성이 더 많이 발생하는 것으로 나타났으나 골절위치에 따라서는(t= 0.293, P= 0.772)로 유의한 차이가 관찰되지 않았다. 하지부동은 평균 6.39 mm (범위, 0.37–15.68 mm) 과성장을 보였으며 이 중 40% (12예)는 15 mm 이상의 과성장을 나타냈다. 5 mm 이상의 과성장이 나타난 경우는 50% (15예)로 환아의 나이는 모두 9세 미만이었으며 통계적 분석결과 환아의 연령이 하지부동에 미치는 결과는 연령이 낮을수록(t=-3.483, P= 0.002) 하지부동이 잘 발생하는 것으로 관찰되었고 골절형태(t=-0.358, P= 0.724)나 골절의 위치(t= 0.005, P= 0.996)와는 유의한 관련성이 없는 것으로 나타났다(Table 1).

고 찰

대퇴골 간부골절은 상완골 과상 골절, 원위요골 골간단 골절, 쇄골골절과 함께 소아에서 가장 흔한 골절 중 하나이며[1] 소아의 대퇴골 골절의 치료 알고리즘에 따르면 6개월 미만에서는 파블릭 장구로 치료하고 6개월에서 6세 사이에는 수상 석고 고정을 시행하며 6세에서 10세 사이에는 유연성 골수내정, 10세 이상에서는 강직성 골수내정을 이용한 수술적 치료를 시행한다[2-4,13]. 유연성 골수내정을 이용한 내고정술 적용이 추천되는 연령은 6–11세, 4–12세, 5–11세 등으로 문헌에 따라 이견이 있으나[13-15] 유연성 골수내정을 이용한 고정술은 수상 석고 고정을 시행한 환자군에 비해 평균 3주 가량 앞서 조기 보행이 가능하며 이로써 위생관리나 빠른 사회복귀를 통해 사회적 고립을 예방할 수 있는 등 이차적인 효과를 볼 수 있다는 장점이 있어 본 연구에 포함된 4세에서 6세 사이의 환아에 대해서도 석고 고정 대신 수술을 통한 치료를 시행하였다.
강직성 골수내정이나 금속판을 이용한 내고정술은 골성장을 저해할 가능성이 있으나 유연성 골수내정은 이와 같은 특성을 갖지 않으며 유연성 골수내정을 이용한 내고정술 시행 시 유연한 움직임으로 인해 조기에 더 많은 양의 가골이 형성된다[16].
현재까지도 유연성 골수내정을 이용한 수술 시 이상적인 정복방법에 대해서는 논쟁이 있다[16-18]. 일부 연구에 따르면 집도의는 수술 중 불필요한 방사능 노출로부터 의사와 환자를 보호할 필요가 있고 다른 종류의 골수내정과 마찬가지로 유연성 골수내정 역시 수술 중 방사능 노출시간이 길어지게 되며 집도의는 장기간의 방사능 노출로 인한 암 발생위험의 증가에 대해 인지해야 하며 필요시 비관혈적 정복술에서 관혈적 정복술로 술기를 전환해야 한다[19,20]. 그러나 관혈적 정복술은 상처감염과 골절치유의 속도를 저하시킬수 있다. 전위가 심한 일부 횡골절이나 강한 근력으로 인해 중첩이 심한 골절, 골절간격에 연부조직이 감입된 경우 도수정복을 이용한 비관혈적 정복술이 어려울 수 있다[17]. 반복적인 비관혈적 정복술은 골절부 주변의 연부조직과 혈관, 신경에 손상을 유발할 수 있으며 궁극적으로 골절치유를 지연시킬 가능성이 있다.
골절발생 시 국소적인 충혈이나 외상으로 인한 성장인자 분비 등으로 인해 과성장이 발생하게 되며[21-23] 소아에서 골 말단의 과성장은 연령이 어릴수록 더 흔히 나타나는데, 이는 어린 소아의 골단에서 성장 잠재력이 더 높기 때문일 것으로 생각되며 상완골, 비골, 경골, 대퇴골, 요골, 척골 순으로 빈도가 높다[24]. Reynolds 등[25]에 의하면 유년기의 대퇴골 간부골절 이후에 사춘기나 이른 성인기까지도 하지부동이 발생할수 있으며 0.5 cm 이내의 하지부동은 장기간에 걸쳐 교정될 가능성이 있지만 5년 간의 추시기간 동안 완전한 교정이 된 경우는 없었다고 보고하고 있다[26,27]. 또한 Sagan 등[28]에 따르면 유연성 골수내정이 삽입되는 방향에 따라 각 형성에 영향을 받으며, 2개의 유연성 골수내정을 삽입할 경우 적어도 1개의 유연성 골수내정은 전면부를 향하도록 삽입할 때 유의하게 각형성 발생률이 감소한다고 하였다. 통계적 검정결과 골절의 위치는 하지부동 및 각형성과의 명확한 인과관계는 관찰되지 않았으나 골절형태에 따라서는 횡골절에서 사골절보다 각형성의 유의하게 더 높은 확률로 나타나며 연령과의 관계에 있어서는 어린 나이의 환아에서 각형성과 하지부동이 유의하게 더 자주 나타나는 것으로 보이고 추시기간 내에 각형성은 교정이 되는 양상이 관찰되나 하지부동에 있어서는 성장이 완료된 이후 2 cm 이상 존재할 경우 신발 굽 높이기, 골단판억제술, 골단축술, 골연장술 등의 추가적인 조치가 필요할 수 있어 이러한 경우 더 신중한 추시를 요할 것으로 생각된다.
유연성 골수내정을 이용한 내고정술 시행 시 9세 미만의 소아에서 횡골절의 형태를 띠는 경우 각형성 및 하지부동의 발생비율이 높고 장기적으로도 교정되지 않을 가능성이 존재하므로 추시기간을 성장이 완료된 이후까지 장기적으로 설정할 필요가 있다.
